When I install a service pack or fixpack on a CMS server, does this modify/expand/molest the repository database? Or does it only copy new files to the server?

Repository structure is not modified.

Your repository will remain untouched. Most of the changes are made to .dll files and the supporting files on your server. if you have a clustered system, you will need to apply the FixPack/Servicepack on each server.
