Request processing failed. (XLS 000009) Error

I have a BO4.1 dashboard which is built using queries against a unx. It was working fine for months, and now out of the blue it is suddenly giving the error "“Request processing failed. (XLS 000009) Create instance of Provider failed”. Does anyone have any ideas? I tried restarting the Dashboard services via the CMC, but no joy. Thanks.

Can you provide some more info? How many different type of Data Conneciton into the dashboard? cached data or on demand? is the universe working fine or had changes made to it? can you replicate that data in QaaWS query or webi just to see if all those object are still working accordingly?
