DAS error - Cannot Refresh Document

After running successfully for almost 2 months, we recently began
experiencing the following failure message when submitting documents to DAS:

‘Cannot Refresh Document’ (error message in DAS log)

‘Command failed Cannot find the universe that uses the
connection.:[]The following data providers have not been
successfully refreshed: .:[]’ (error message in document
tmp file)

I have been working with BO Tech Support on this issue and at their request,
have changed all of connections to ‘Disconnect after each Transaction’, and
recreated our BOMain.key. Additionally, I have performed a scan and repair
on the repository domains. However, I still receive the same error. BO also
suggested upgrading to 4.1.4; however, that also has not resolved the
problem.

The error is not consistent based on user or document: those reports that
fail, will run successfully if resubmitted to DAS or run interactively by
the same user. Also, our DAS is solely dedicated to BO and has plenty of
memory, etc. so I don’t believe that that is the cause.

Our configuration:
BO v 4.1.3 w/ service pack CSP22
Oracle 7.3 database

This is driving me crazy, so if anyone has experienced this same message or
has any suggestions/solutions, I would greatly appreciate the help.

The problems you are having can be because of the following
reasons:
1) Running low on Memory on the Doc Agent Machine. This
happens to us some times. Even tough we have 198 MB of RAM (ours is also
a dedicated server) for 12 tasks, sometimes the machines runs low on
memory because of help up tasks or all 12 tasks running on the machine.
The solution will be to reboot the machine once every week so
that the held up tasks get cleaned up.
2)Dr. Watson error on the Doc Agent Server. If one of the user
has submitted a query that resulted in a Dr.Watson error unless some one
cleans it from the machine, the jobs will continue to fail will the
error “Cannot Refresh Document” (BO Tech Support has not been able to
reproduce this problem, but I encounter it too often).
3)If the Primary Doc Agent is shut down then all the Jobs that
were running will have the status “Cannot Refresh Document”.
