Need help with best practices in gathering report req's

I am looking to create a template to use as a Report Requirements Doc that my end users can use to put their requirements for a new report in. I am hoping this will eliminate the problems that come with verbally describing what they want in a report.

Have any of you created/found a good document that would ask all the right questions and provide the user a way to clearly document their reporting requirements? and be willing to share it?
