Multi value cascading prompt

Hi,

I have a request for cascading prompt.
Users have to enter 4 values to get their related Request IDs from the second prompt screen
Example:

First Prompt Screen:
1- Requestor: John
2- Request Name: Database
3- Request Type: Install
4- Service Type: DB Install

Second Prompt Screen Object
1- Request ID:

LOVs EG234
AB543
DE890
.
…

Any idea how can I develop this type of cascading prompt?

BO6.5
Oracle
